Ask any doctor who’s been practicing for longer than 20 years and he or she will tell you that it began with the rise of HMOs and managed care in the late 1980s. But to prove the anecdotes one would have to get data, and to be honest, simply finding the data for the 90’s wasn’t easy so I can imagine what it would be like to search for earlier data.
